The elder Rakowski’s death may have been avoidable if the information in his medical records were accessible by the team at Bridgeport Hospital, according to the founders at SAMI Health, a Westport, Conn.-based healthcare IT business that was formed to be sure medical errors like Rakowski’s are avoided.
